Sick/Fit Note Certificates

Self Certification Sick/Fit Note for less than 7 days

If you have been ill for 7 days or less you do not need to see a Doctor. You can complete a Self-Certification form yourself.

However some employers insist on a Doctor’s note regardless. A Doctors note under these circumstances involves a charge of £10.

Many employers have their own self-certification forms. If your employer doesn’t have its own form you can download the Self Certification Form. Please print it, fill it in and hand it in to your employer. 

 

Doctor’s Sick/Fit Note for more than 7 days

If you have been ill for more than 7 days, you will need a Fitness to Work certificate. To request a certificate, please use the link above and submit an admin request through PATCH'S.

Please note that we have a professional duty to review all Fit Note requests in accordance with government guidelines. A clinician will review your request, and we will be in touch to advise you of the outcome. You may be issued a Fit Note, or you may be invited for a consultation with a clinician, either by telephone or face-to-face.

 

Requesting an additional Sick/Fit Note

If you require an additional Sick Note (Fit Note) for your illness, you can submit a Fit Note extension request through PATCHS using the form above. A Clinician will review your request and will action what is necessary.
